I understand that my child must obey all established rules and follow the instructions of the person in charge from Sabeel Muslim Youth & Community Center.
I acknowledge that there are certain risks associated with certain Masjid Activities. Also, I acknowledge that there may be other risks inherent in these activities of which I may not be presently aware.
I further acknowledge and confirm that I am solely responsible for the accuracy of all information provided in the registration form, including but not limited to my child’s date of birth and age. I understand that the Sabeel Muslim Youth & Community Center relies on this information for proper placement and supervision. In the event that any information is found to be inaccurate or misleading, particularly regarding my child’s age, I agree that the Center reserves the full right and discretion to reassign my child to an appropriate age group if available, or to remove my child from the program or camp without entitlement to refund or compensation.
By signing this parental consent and liability form, I expressly warrant that my child named above is capable of withstanding both the physical and mental demands associated with the Sabeel Muslim Youth & Community Center activity for which he is registered. I also expressly assume all risks to my child’s participation in these Sabeel Muslim Youth & Community Center activities, whether such risks are known or unknown to me at this time. In recognition of these risks and realities, and in consideration of my child being offered the opportunity to participate in and benefit from Masjid Activities, I release, waive and disclaim all liability, claims, and demands of whatever kind or nature, either in law or equity, which arise or may hereafter arise in relation to my child’s participation in the activity at Sabeel Muslim Youth & Community Center.
I understand and acknowledge that this release discharges Sabeel Muslim Youth & Community Center from liability for any claim that I may have against Sabeel Muslim Youth & Community Center with respect to bodily injury, personal injury, illness, death, and/or loss or damage of property, court costs, attorneys’ fees and interest as a result of my child’s participation in the Sabeel Muslim Youth & Community Center activity.
I, the undersigned, certify that I am the parent or legal guardian of the participant mentioned above. I have read this consent carefully and agree to its terms. I hereby authorize my child named below to attend and participate in the Masjid Activities, including any off-site activities for which I have registered my child.

Cancellation & Refund Policy
In the event of a cancellation request, 10% of the paid amount will be deducted to cover payment processing fees and administrative costs.
No refunds will be issued if the withdrawal request is made within seven days or less before the start date of the camp.
Cancellation requests must be submitted by email to:

By completing the registration, the participant/guardian acknowledges and agrees to the cancellation and refund policy stated above.
